On Thursday, February 18th, the state government launched the Minnesota COVID-19 Vaccine Connector to let residents know when, where, and how they can get the vaccine. This free online tool streamlines planning for the vaccination of people 18 years, regardless of where they live, their immigration status, personal situation and vaccine eligibility.

To stay up to date on your eligibility and find out about opportunities to get the COVID-19 vaccine in your area when available, fill out the online form, available on the State of Minnesota’s website: vaccineconnector.mn.gov/.

Signing up for the Vaccine Connector is free, easy, and secure.

  • There is no time limit to get registered.
  • You do not need to provide health insurance information.
  • You do not need to provide an ID.

To sign up, follow these three steps:

  1. Complete the online form by clicking here.
  2. Once you’ve completed the form, you’ll receive an email confirming that your registration was successful.
  3. When you become eligible to receive the COVID-19 vaccine, you’ll receive information about vaccine opportunities in your area and how to schedule an appointment. If you don’t have an email address, you’ll be notified by text or phone call.

If you have questions about the form or need language assistance, please call our Minnesota COVID-19 Vaccine Connector hotline at 833-431-2053.

What information will I need to provide to sign up?

  • Demographic information: race, ethnicity, occupation, sex and age.
  • Medical history: illnesses or chronic conditions.
