Minnesota is rewarding people who get vaccinated against COVID-19

Get vaccinated and claim your reward from a new summer program through the State. The first 100,000 Minnesota residents 12 and older who get their first dose of the COVID-19 vaccine between May 27 and June 30, 2021 will be eligible for a reward.  The rewards request form is available at this link and must be submitted by 11:59pm on June 30, 2021. As clarified within the link, vaccination information is subject to verification by the Minnesota Department of Health.

Once you’ve received your first dose, you can also call (833) 431-2053 from Monday to Friday between 9 a.m. and 7 p.m. to record your information. Assistance is available in several languages.

 

Available rewards include: a $25 Visa gift card, a pass to the Great Lakes Aquarium, entrance to Valleyfair or Mall of America’s Nickelodeon Universe, admission to the zoo, and many more. For a complete list of rewards and further details about this program, visit  this link.  

To complete the form, you must be 18 years or older at the time of the vaccine. If you are a minor, you can have your form completed by a parent or guardian.

Reward yourself with good health while helping and protecting your community! Minnesota is on track to reach its goal of vaccinating 70 percent of residents 16 and older by July 1st.
